Prevalencia del Conducto Medio-Mesial en Primeros Molares Inferiores en una Población Argentina

El objetivo del presente estudio fue determinar la prevalencia del conducto medio mesial en primer molar mandibular permanente y sus características anatómicas mediante tomografía computada de haz cónico (CBCT). Materiales y métodos: Se incluyeron imágenes de CBCT de 300 primeros molares mandibulares de 190 pacientes atendidos en la Cátedra de Endodoncia de la Facultad de Odontología de la Universidad de Buenos Aires. Las imágenes se analizaron mediante alineación tridimensional y evaluación en los planos axial, sagital y coronal. Para describir variables categóricas se calcularon porcentajes con intervalos de confianza al 95% (IC95). Se utilizó la prueba exacta de Fisher para analizar asociación (p < 0,05, significativo). Resultados: El conducto medio mesial estuvo presente en 4 (1%; IC95: 1% a 3%) de los 300 dientes observados. Dentro de los 4 conductos medios mesiales encontrados, hubo 3 confluentes y uno independiente. No se detectó asociación significativa entre la posición del diente (3.6 o 4.6) y la presencia del conducto medio mesial (p = 0,37). Tampoco hubo asociación significativa entre el sexo del paciente y la presencia del conducto medio mesial (p = 0,32), ni entre presencia y grupo de edad (hasta 40 años) (p ≈ 1). Conclusión: Los resultados indican que el conducto medio mesial es una variación anatómica poco frecuente en los primeros molares mandibulares, aunque clínicamente relevante, por lo que el conocimiento detallado de la anatomía interna, y el uso de herramientas diagnósticas resulta fundamental para mejorar la detección de esta variación anatómica y optimizar la planificación del tratamiento.
